Peyronie’s disease is characterised by the stiffening of tissue (fibrosis) in the penis, leading to penis deformity. On the inner lining of the erectile tissue, which holds most of the plasma in the penis throughout erection, a lump or plaque (scar tissue) appears. A firm lump can usually be seen in the area where the penis bends. The hardened region, or plaque, prevents the erect penis from expanding normally, affecting its size and form. In severe situations, the plaque might encircle the penile muscle and arteries, causing erectile dysfunction.

What are the symptoms of Peyronie’s disease?

Peyronie’s disease (PD) begins as a minor enlargement or inflammation on the top or bottom side of the penis that develops into a lump. It normally develops over time, although it can also arise suddenly. Some men develop PD as a result of a penile injury. These men frequently report hearing a loud “crack” and experiencing discomfort and bruising in their penis.

Men, on the other hand, usually have not reported any injuries. When aroused, PD can be uncomfortable, impair flexibility, and in most cases, it can lead to shortening or bending of the penis. In certain situations, the penis may have an hourglass malformation or rigidity. Some men have trouble getting or maintaining an erection. In other situations, the penis hardens just up to the scar and remains flaccid (not erect) beyond that point.

The Causes Behind Peyronie’s disease

Although the precise cause of PD is unknown, it is supposed to be triggered by frequent bending or hitting of an erect penis. The erectile tissue lining breaks, resulting in an inflammatory coating (scar tissue) that does not heal properly. Blood flow can be disrupted if tiny blood vessels erupted. This type of damage usually causes swelling and/or inflammation (redness and/or heat) and heals on its own within a year. However, some men take years to recover, and bruising can develop. A pre-existing immunologic disease or diabetes may cause PD after a minor penile injury. Men are more likely to get Parkinson’s disease after puberty, yet the most prevalent age range is 45 to 60 years old.

The Impact of Peyronie’s disease on sexual intercourse

Some men may have penile pain when getting an erection in the early stages. The effect on intercourse is typically mild in moderate cases of PD where the lump does not force the penis to bend too much. Moderate to severe PD, on the other hand, can make intercourse impossible due to the shape of the penis or difficulties achieving an erection.

On the basis of medical history and physical examination, a male sexual health expert may typically identify PD. When the penis is flat, lumps can be seen and felt. The doctor will have to check how much the penis is bending if it is not hard.  Oftentimes, a doctor can conduct an OPD-based intracavernosal shot (penis injection) that can result in arousal in minutes, allowing the doctor to accurately measure the penis’ curvature, which will enable him in taking corrective action.

The Treatment Of Peyronie’s disease

The severity of the curvature, the level of inflammation present, the difficulty in having sexual intercourse, and how much it affects the person are all factors considered at the time of treating Peyronie’s disease.

If the deformation is not severe and the pain is tolerable with pain relievers, the patient can wait and observe if the bending increases or not. Depending on the availability of medications, medical therapy may involve the administration of anti-inflammatory, vitamin E, and potassium para aminobenzoate. These medications may or may not improve the curvature, but they can help to minimise plaque sensitivity.

For patients who have had Peyronie’s disease for a long time and have a consistent deformity, surgery is usually the best option. Shockwave therapy is a new treatment option for people with modest curvatures that is non-invasive, painless, and medication-free.

The Frequency Of Peyronie’s disease

Adult males who do not identify the disease, are too ashamed to see a doctor, or do not seek medical care considering the symptoms are mild, so the exact number of men with PD is unknown. According to recent demographic research, one out of every ten men has Parkinson’s disease. In Western countries, it is more common.
